Following extensive consultation to determine the service response that would best meet the needs of people on the Gold Coast who are living with severe and complex mental illness, Gold Coast Primary Health Network (GCPHN) would like to invite you to hear more about our upcoming funding opportunity.

GCPHN are here as advocates of people with severe and complex mental illness. We want to co-create service solutions with you so that the positive experiences in people's life are sustained, and bad experiences are mitigated. We don't have a monopoly on solutions but know there is no one size fits all approach when providing services for people living with severe and complex mental illness.

We want organisations to develop and present innovative solutions to deliver three service components that will meet people's needs, now and into the future. Creating an environment that builds the confidence and capability of the consumer and their family to manage their own health is important to us and will be an important element of the service solution that organisations develop in response to this funding opportunity.

The three service components include:
* Intake, triage, service information and referral options/pathways
* Clinical care coordination, and
* Community based safe space

Continuity of care for the consumer, integration across providers and shared care planning between clinicians are fundamentals in this service response and each component will work in alignment with the other.
