Gold Coast Primary Health Network (GCPHN) is seeking responses for a Request for Proposal (RFP) for services for people with severe and complex mental illness to assist in their recovery journey.
This RFP supports and encourages innovation and provides the space for non-traditional models of service delivery to be considered to meet consumer needs.
The following service components that form the basis of this RFP are priority areas that were identified through the recent GCPHN co-design process and are required in a service response to meet the needs of people with severe and complex mental illness.
SERVICE COMPONENTS:
1. Service information, referral pathways, triage and intake across the stepped care model
2. Clinical care coordination, and
3. Community based safe space.
