Gold Coast Primary Health Network (GCPHN).
GCPHN has an opportunity for a lead agency to establish a Complex Needs Assessment Panel (CNAP) for individuals aged 35+ with complex drug and/or alcohol, psychosocial and physical health needs who continue to fall through service gaps and cannot have their complex needs met by one service or agency.
The need for a stand-alone CNAP for drug and alcohol was identified through consultation undertaken by GCPHN throughout 2017. The current CNAP services (where mental health needs to be the primary diagnosis, or where the age limit is under 35 years) that operate on the Gold Coast cannot be accessed by the identified target group due to existing eligibility criteria, and they continue to experience barriers in accessing coordinated and comprehensive services to support their complex needs.
CNAPs aim to improve and integrate service delivery to achieve positive outcomes in overall quality of life and health. This is achieved by addressing issues and barriers the person is facing through a flexible approach to case planning and the implementation of ongoing holistic interventions.
