Gold Coast Primary Health Network (GCPHN) is seeking to partner with organisations to facilitate an approach to the development of needs assessment methodology, planning, co-design and commissioning of high quality, accessible and cost effective aged care and palliative care pathways and services in the Gold Coast region that are person, carer and community centred.
The regionalised methodology, summary needs assessment and blueprint documents will need to be developed in a way that facilitates alignment with planning, stakeholder engagement, commissioning and service co-design cycles to enable delivery of regionally agreed, evidence informed and a sustainable approach.
The selected provider will also need to provide an outline of how work will be undertaken in a way that collaboratively engages an identified working group of key Gold Coast PHN staff that will be responsible for progressing implementation of the signed off regionalised methodology and blueprint.
