Gold Coast Primary Health Network is seeking a consultancy provider to scope, design and provide implementation guidance regarding the development of two projects and localised methodologies for:

1. Agreed shared care for the Gold Coast health system (across primary and acute care), supported by a high-level implementation program and an initial exemplar palliative care shared care arrangement.

2. Develop agreed health pathways for the Gold Coast health system (across primary, community and acute care), supported by a high level implementation program and the initial exemplar palliative care pathway (and potentially Alcohol and Other Drugs treatment services) and forward plan.
