Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) is a commissioning organisation and is inviting Service Providers to undertake a pre-qualification questionnaire (PQQ) for eligibility for membership on a Mental Health Clinical Service Providers Panel (the Panel) across Central Queensland, Wide Bay and the Sunshine Coast.

This Response Form is for Organisations to respond to the PQQ. There is a separate PQQ Response Form for Sole Practitioners.
For the purpose of this PQQ a Sole Practitioner means an individual who is registered and licensed to practice as a Sole Practitioner under their professional registration conditions. Please note, that while Sole Practitioners may be registered as a company, this response form applies to individuals, who are practicing alone, are not in a partnership with other practitioners and do not employ or sub-contract clinical staff in their practice.

The objective of this PQQ opportunity is to establish a Mental Health Clinical Service Provider Panel across the PHN region, where successful providers will enter into a Master Service Agreement with the PHN. Once the Panel is established, the PHN will invite interested Panel members to participate in either a request for quote or request for proposal for the new funding for Mental Health in-reach into Residential Aged Care Facilities. The PHN will also be in a position to call on Panel members to provide quotes or proposals or make direct approaches to Panellists, as other additional mental health funding becomes available in the future.
Eligibility: Companies or other legal entities who can form legally binding contracts; are over the age of 18 years; and hold an ABN.

Proposed Term: The proposed term during which successful Panellists are eligible to receive Work Orders from the PHN is up to 30 June 2022.
