SNSW LHD is seeking a single provider to ensure TACP service provision across the Goulburn Mulwaree Council and Upper Lachlan Shire area is delivered in the manner most clinically effective, administratively efficient, cost effective / best value and as judged on other evaluation criteria for the RFQ, including, but not limited to, the Tenderers performance, reliability, capacity and compliance with relevant standards.

The contract period shall be for one (1) year with an option to extend for a further one (1) x 12 month period dependent of Government funding and mutual agreement to extend. All contracts arising from this RFQ are conditional on SNSW LHD receiving funding from the Australian Government Department of Health Transition Aged Care Program.
