WentWest is currently seeking Expressions of interest to supply Remote Health Monitoring Devices.
The remote monitoring devices must be capable of monitoring heart rate (HR), blood pressure (BP) and Electrocardiogram (ECG) readings to assist with screening and centralised monitoring of patients at risk or diagnosed with cardiovascular disease.

The device must be able to feed data back to CareMonitor SaaS. CareMonitor is the shared care, real-time remote monitoring and population health management platform. Patient Centred Medical Homes and Healthcare Neighbourhoods will use CareMonitor to provide seamless integration with a range of clinical management systems enabling coordination of complex clinical pathways, care planning, medication management and patient engagement.

This expression of interest relates to the roll-out of the Cardiology in the Community (CIC) model of care through the Collaborative Commissioning Program. The objective of this model is to deliver integrated and coordinated care in the primary health care setting, with appropriate access to, and support from the acute and community care sectors - delivering the right care, at the right time, in the right place - the first time.
