Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) has identified geographical areas within their service delivery region, with high rates of diabetes and limited access to a Credentialed Diabetes Educator (CDE). Local service providers have identified that a barrier to increasing diabetes education services within the region has been attaining the 1,000 practice hours that is a requirement of the credentialing process. Therefore, the PHN is offering support to a limited number of Health Professionals to complete their 1,000 practice hours and achieve their credentials.
The PHN is inviting interested Health Professionals, who have completed or are undertaking a graduate certificate in diabetes education and management, to submit an EOI for the CDE program.
Target areas are:
- - Banana Shire (excluding Theodore);
- - Gladstone; or
- - Central Highlands regions.
Once credentialed, Bidders must be able to continue to provide Diabetes Education services within one of the regions outlined above, with full support of a Primary Health Care facility, funded by MBS or private billings.
Funding will be provided to primary health care services that employ the prospective CDE. They will need to ensure that the prospective CDE complies with the contract in delivering services within the contracted, time, cost and scope.
Eligible applicants who are not employed by a Primary Health Care facility are still encouraged to apply.
Eligibility: The EOI is limited to Health Professionals who have commenced or completed a Graduate Certificate in diabetes education and management and who work in the target areas.
