Central Queensland, Wide Bay, Sunshine Coast PHN (the PHN) has identified a need for an Active at Home project within the PHN catchment area, to support people aged over 65 years (Indigenous people aged over 50 years) to exercise safely and regularly within their own home.
Eligibility: This Expression of Interest is ONLY open to service providers who currently provide in-home care services to clients aged over 65 years (Aboriginal clients aged over 50) within the PHN catchment area. The Active at Home service is for eligible clients as part of their established in-home care program.
The Active at Home program has been developed in association with the Australian Government and strongly aligned with the aims of national aged care reforms. The program offers a cost-effective solution for home care providers, enabling them to make a real difference to the health and wellbeing of their ageing clients, while also meeting Government requirements to deliver (and report on) wellness and reablement services. The objective of the Personal Care Worker (PCW) led, 12-week home exercise program enables is to help older people regain mobility for continued independence as they age. The initiative supports older people to participate in the program as part of their current in-home service and by a PCW who has undertaken the accredited training.
