As part of the Federal Government's response to the Royal Commission into Aged Care Quality and Safety, the Government has announced funding for PHNs to commission early intervention initiatives to support healthy ageing and assist chronic disease management in the community.
Frailty has been identified as a key health issue affecting older people living in the community across the Brisbane South region. Frailty is an age-related condition that is characterised by functional decline, fatigue, slow motor performance, and cognitive impairment that results in an increased vulnerability to dependence, adverse health outcomes and mortality. People who are frail have 2 to 3 times the health care utilisation of their non-frail counterparts, with substantial individual and community-level health care costs driven by inpatient, pharmaceutical and long-term care costs and reduced patient quality of life.
It has been recognised that frailty is not an inevitable consequence of ageing and the potential for early detection and remediation exists. The progression of frailty and functional decline can be curbed by participation in multi-component frailty intervention programs.
Brisbane South PHN (BSPHN) is seeking a provider/s that can deliver a multi-component frailty program to reduce or decline the progression of frailty in older people living in the community and slow down the common problems associated with ageing.
BSPHN is seeking to engage a provider who has demonstrated knowledge, expertise and clinical/communication systems to operationally and strategically deliver a mutli-component program and has a willingness to work in close partnership with BSPHN to co-design, deliver and evaluate the model to ensure older people experience a greater quality of life through safe, connected, and coordinated person-centered care.
