Tender Description
WSPHN is intending to provide funding to one or more providers to operate as care finders in Western Sydney Region with no option for sub-contracting. However, we encourage the successful organisation/s to develop partnerships and referral pathways with services to provide holistic support for the clients.
The objective of the Care Finder program is to establish and maintain a national care finder network that provides specialist and intensive assistance to help people understand and access aged care and connect with other relevant supports in the community.
