ABOUT THE REQUEST FOR Tender (RFT): Melbourne Primary Care Network (MPCN) trading as North Western Melbourne Primary Health Network (NWMPHN) is undertaking a public open tender for the provision of care finder services in the north western Melbourne region.
NWMPHN will commission service providers to establish, deliver and maintain a network of care finders to provide specialist and intensive assistance to help people within the care finder target population.
