This proposal is seeking the establishment of a network of care finders to provide aged care specialist and intensive assistance services. The care finder program was announced in the 2021-22 Budget as part of the Connecting Senior Australians to Aged Care Services measure and will be delivered through PHNs. Care finders will:

- provide specialist and intensive assistance to help people to understand and access aged care and connect with other relevant supports in the community and;

- specifically target people who have one or more reasons for requiring intensive support to interact with My Aged Care (either through the website, contact centre or face-to-face in Services Australia service centres), access aged care services and/or access other relevant supports in the community.

Brisbane South PHN will establish and maintain a network of care finders who will provide specialist and intensive assistance to help people within the care finder target population to understand and access aged care and connect with other relevant supports in the community.

Prospective Tenderers will receive an official invitation to market briefing which will occur on Thursday 6th of October 2022 from 11:30am to 12pm.
