As part of the Australian Government's response to the Royal Commission into Aged Care Quality and Safety, Country SA PHN is seeking responses to this Request for Tender (RFT) from suitable organisations to improve outcomes for senior Australians through support to access primary care and aged care services.

Applicants are invited to outline their approach to the Care Finder Program that will provide specialist and intensive assistance to help people within the Care Finder target population to understand and access aged care and connect with other relevant supports in the community.

The contract term is 1 January 2023 to 30 June 2025, with program commencement from 1 January 2023 and full implementation by 30 April 2023.
