Gold Coast Primary Health Network (GCPHN) is seeking responses for a Request for Proposal (RFP) from eligible organisations to provide specialist and intensive assistance specifically to older Australians to help them understand and access aged care supports.
The Care Finder Program was announced in the 2021-22 Budget as part of the Connecting Senior Australians to Aged Care Services measure and will be delivered through PHNs. Care Finders will:
- provide specialist and intensive assistance to help people to understand and access
aged care and connect with other relevant supports in the community.
- specifically target people who have one or more reasons for requiring intensive support
to interact with My Aged Care (either the website, contact centre or face-to-face in
Services Australia service centres) and access aged care services.
REGISTRATION: Proposal documents can be downloaded after registration has been completed

SUBMISSIONS: Completed responses can ONLY be submitted via the evaluation response form. Respondents will receive a Successful Submission Receipt timed and dated upon completion.
