ABOUT: Sydney North Health Network (SNHN) is inviting suitably qualified and experienced providers to apply to deliver early intervention program(s) for older people in the Northern Sydney community. The service will target older Australians and will deliver activities that promote healthy ageing, slow decline and support the ongoing management of chronic conditions in the community.
