WVPHN is inviting proposals from eligible service providers to implement and deliver the Regional Care Partnerships: Chronic Conditions service model, targeting areas in most need within the WVPHN region.

The Regional Care Partnerships: Chronic Conditions service model will see the recommissioning of a more sustainable, integrated primary care service model delivered through strong partnerships and driven by collaborative care team approaches. Within the model, clients will move between their usual primary care provider, commissioned Suppliers and other service providers for treatment interventions tailored to their needs and preferences.
