ABOUT: Sydney North Health Network (SNHN) is inviting suitably qualified and experienced providers of virtual care solutions to apply to deliver the SNHN Residential Aged Care (RACF) Facility Virtual Care Service.
The SNHN RACF Virtual Care Service is a clinical support service focused on providing virtual care capabilities (including secure videoconferencing and remote vital signs monitoring) to RACFs and their healthcare providers, to enable timelier access by RACF residents to their primary care providers and other clinicians. The Service is also aimed at improving data sharing between RACFs, primary care providers and acute care sector. The Service will include support to RACFs in the form of virtual care hardware, software and ongoing education/technical support.
