Brisbane North PHN is inviting tenders from eligible organisations to deliver a social prescribing service as a proof-of-concept in the Caboolture/Kilcoy hospital catchment, commencing towards the end of March 2023.
Social prescribing consists of a referral or connection of someone to a Link Worker, where they work together to explore opportunities and co-produce a plan of non-medical activities and social supports aligned to their needs and interests. That person is then supported and empowered to access the resources available within their own community to improve their social, emotional and physical health and wellbeing.
A social prescribing program provides an opportunity to address some of the social determinants contributing to poor health and wellbeing, particularly for those experiencing social isolation and loneliness.
The PHN intends to procure a social prescribing service with the aim to:
- Decrease isolation and loneliness and reduce health risks associated with it
- Increase physical activity
- Increase mental stimulation and reduce risk or progression of dementia in older people
- Improve mental health and wellbeing
- Reduce GP visits and hospital admissions.

An online one-hour sector briefing session will be held for anyone interested in tendering for this service. The session will provide:
- a summary of the intergenerational program
- an outline of the tender process and relevant dates
- the opportunity to ask questions.
The online sector briefing is scheduled for 3pm (Brisbane time), Monday 16 January 2023 - Refer to Attachment 8 for connection details.
Attendance at the sector briefing is not mandatory. A recording of the session, and summary of questions and answers from the session will be available from the tender page following the conclusion of the session.
