The Australian Government is investing $1 million annually for the management of chronic conditions in the Darling Downs and West Moreton PHN (DDWMPHN) region between 1 July 2023 to 30 June 2025. The initiative seeks to address service gaps for the provision of accessible and cost-free services to people in underserviced populations.

In 2017, The Australian Health Ministers' Advisory Council released the National Strategic Framework for Chronic Conditions which identifies coordinated and person-centred care as a key objective in the management of chronic conditions. It is acknowledged that Australians are living longer, being diagnosed earlier and have complex care needs. There are also specific vulnerable clients who are at greater risk of developing chronic disease and who need additional and targeted support to address these health needs.
