Tender Description
Murray PHN is releasing a request for proposal (RFP) to fund a pilot program to support individuals with an Intellectual and or physical disability.
The goal is to improve access to primary care services for the eligible population, the outcome is to improve the health of the eligible individuals with the management of chronic and complex issues as well as early intervention and management of predisposing factors.
Funding will be released to one general practice with expectations to develop and to pilot a model of care.
