Tender Description
This initiative is a collaboration between Primary Health Tasmania and the Department of Health, Tasmania.
Grants of up to $100,000 are available to eligible organisations to propose an activity to review, modify and/or research care models and undertake an evidence-based redesign process to improve the outcomes of patients with life-limiting illnesses. The redesign process may involve, but is not limited to:
- research, design, and implementation of innovative service models, including workforce support
- integration of existing services through the use of alternate patient pathways
- improving existing and developing new collaborations across the palliative care sector including consumers' primary care, community care, hospital care, and specialist services
- modification of policies and procedures to support innovative models.
Online information sessions
A number of online information sessions will be offered. Interested parties are welcome to join any and/or as many of the information sessions as they like. Dates, times, and links to each of the sessions are listed within the palliative care grant guidelines.
How to apply
You need to download and complete the application form. Once completed upload the application form via this eTender Portal (refer to Attachment 1 - responding to a tendering notice) along with any relevant documents which can be used to substantiate grant application.
Grant timeframes
Grant activity will commence in July 2023 through until June 2024.
