ABOUT THE REQUEST FOR Tender (RFT): Melbourne Primary Care Network (MPCN) trading as North Western Melbourne Primary Health Network (NWMPHN) is undertaking a public open tender for the Early Interventions for Healthy Ageing project in the north-western Melbourne region.

NWMPHN will commission general practices to develop and implement a project to test an enhanced model of care that complements existing Medicare Benefits Scheme (MBS) funded chronic conditions prevention and management activities.
