The Primary Health Network (PHN) has received funding to empower older Australians and promote their independent lives in communities for as long as possible. This initiative focuses on proactive and preventative measures that encourage healthy aging, slow down physical and mental decline, and assist in managing chronic conditions. These initiatives align with the recommendations of the Royal Commission and contribute to broader aged care reform.
The primary objectives of the interventions and programs implemented by PHN are as follows:
1. Prevent, identify, and manage chronic diseases and health complications.
2. Minimise unnecessary hospital admissions.
3. Enhance overall health outcomes for the senior population in Australia.
In Western NSW, a Needs Assessment highlighted frailty as one of the top two health concerns within the region for the older population. Additional consultations with community members and clinicians emphasized the importance of fall prevention in enabling people to continue living at home, reducing hospital admissions, and delaying entry into aged care facilities.
