This Request is for the supply of ophthalmic ultrasound system, associated accessories and optional post warranty maintenance and service for the SMHS. The scope of this purchase shall include:

a) the supply, delivery, installation and commissioning of the ophthalmic ultrasound;

b) minimum Twelve (12) month warranty;

c) options for post-warranty service and maintenance for up to Four (4) years; and

d) provision of in-service, education and training for the clinical users / operators, including the correct, safe and efficient operation of the equipment.

Initially, it is anticipated that One (1) ophthalmic ultrasound system is required for Ophthalmology Service at Fremantle Hospital. Additional units may be purchased within the Term of the Head Agreement. 

The Head Agreement established by this Request will constitute a standing offer, whereby the Customer can place orders for additional equipment, associated accessories, consumables and maintenance options offered on an as required basis during the term of the Head Agreement.
