Primary Health Networks (PHNs) have been established with the key objective of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure clients receive the right care in the right place at the right time.

The implementation of Head to Health Adult Mental Health Service (HH-AMHS) sites are designed to help adult Australians experiencing mental health challenges through creating accessible services by reducing the existing fragmentation within the current mental health service system. HH-AMS aims to enhance local mental health care service integration, by offering a seamless care pathway and variety of entry points into mental health care for adults to access and receive the adequate care at the right time to meet their mental health needs.

Country to Coast Qld (CCQ) is seeking proposals from organisations with the experience, capability and innovative vision to establish Head to Health Adult Mental Health Services (HH-AMHS) sites across the region. By this opportunity, CCQ will commission one Centre (Sunshine Coast) and three Satellites (Bundaberg, Rockhampton and Gladstone).
