Eastern Melbourne PHN (EMPHN) invites Proposals from suitably qualified Respondents to:
• Develop recommendations for a model of care that considers the sustainability, scalability, and financial viability of the RCBH program.
• Develop an Evaluation Plan, including data requirements of the RCBH Program based on the sustainability recommendations.
In accordance with the requirements of this Request for Proposal (RFP).
EMPHN aims to grow the Right Care = Better Health (RCBH) Program into a catchment wide sustainable response to chronic and complex condition management in general practice Through this proposal we are aiming to:
1) Improve program sustainability and scalability, by:
a. understanding the funding options available and whether the system can supplement the cost of the program, particularly once EMPHN funding is withdrawn.
b. understanding relevant program costs (unit, episode) against system benchmarks.
c. identifying adjustments to the program that may lower program costs.
2) Demonstrate program outcomes, by:
a. developing a program logic based on the enhanced model of care.
b. developing an evaluation plan that details proposed methodology and data requirements.
c. identifying Key Performance Indicators (KPIs) that will facilitate tracking progress against outcomes.
The objectives for the provider engagement are to:
- assess future funding model options through analysis of the existing model of care
- propose changes to existing RCBH delivery model that may reduce cost and/or improve health outcomes
- develop costing and benchmark method (episode of care) for future use
- develop an evaluation plan and methodology based on proposed RCBH model changes
- work collaboratively with EMPHN and the existing service provider staff to ensure the models and advice are aligned and understood by RCBH teams.
