Tender Description

Firsthealth is federally funded to deliver the Primary Heath Network program across the Murrumbidgee region. Murrumbidgee Primary Health Network (MPHN) is one of 31 local and independent PHNs established nationally to support the primary healthcare sector and improve health outcomes, particularly for people at risk of poorer health outcomes. We deliver the PHN program across approximately 126,000 square kilometres to more than 250,000 people living in our 508 communities.

With the person at the centre of care, we plan and deliver programs to ensure the needs of our individual local communities inform our approaches. We achieve this by working with our community and partners to:

- Coordinate local services and systems to improve coordination of care.

- Commissions primary health services to meet population health needs with a focus on access and equity.

- Support capacity-building to ensure sustainable healthcare workforce through quality improvement, professional development and other innovative measures.

ABOUT THIS REQUEST FOR PROPOSAL

In the recent 2023 Health Needs Assessment (HNA) Department of Health and Aged Care update, MPHN identified evidence that homelessness is an emerging risk in our region.

MPHN is seeking a region-wide Homelessness Health Needs Assessment activity to identify the primary health care needs and barriers to accessing care for people experiencing, or at risk of homelessness. This work will identify options for models of care and inform future co-design methodology to improve primary health care access and health outcomes for this priority group. This activity will include active engagement with key service provider stakeholders including but not limited to, family and domestic violence support services, emergency accommodation support services, and organisations providing assistance to gain and sustain housing, family and relationship services, mental health services and disability services.

OPPORTUNITY ON OFFER

MPHN is seeking proposals from suitably experienced providers to undertake a Homelessness Health Needs Assessment involving an extensive consultation process with homelessness service providers to collect qualitative and quantitative data. The collected data will then be synthesised, and the analysis used to develop a recommendations report to MPHN on innovative and localised models of care within the primary care sector to increase the accessibility of these services for individuals at risk of, or currently experiencing homelessness.
