Eastern Melbourne PHN (EMPHN) invites Tender Responses for the provision of Right Care Better Health (RCBH) in accordance with the requirements of this Request for Tender (RFT).
The Australian Government has established Primary Health Networks (PHNs) to plan and commission a range of primary health services to meet the needs of their population.
The key objectives are to increase the efficiency and effectiveness of medical services to older Australians and people with chronic and complex conditions, particularly those at risk of poor health outcomes, and improve coordination of care to ensure they receive the right care in the right place at the right time.
The Right Care = Better Health (RCBH) Service offers individually tailored care co-ordination for eligible clients with complex chronic conditions. Care coordination is provided by EMPHN-funded community health employed, nurse care coordinators within participating general practices, in partnership with the practice team. The aim is to support general practices to enhance team-based care and utilise shared care practice principles to facilitate service navigation and care coordination for eligible clients.
The program is currently available to clients in the East of the EMPHN catchment. EMPHN is seeking to expand the program in the Northern region in the LGAs of:
- Whittlesea
- Murrindindi (part of LGA)
- Mitchell (part of LGA)
The care coordination model of care, which is already being rolled out in the East of the EMPHN catchment focuses on patients within general practices who suffer with chronic and complex conditions.
This program will provide a targeted care and service coordination program in the geographical areas of highest need to support the improvement of chronic disease management and reduce the risk of early admission to residential aged care.
