NHMRC-GACD 2024: Management of Multiple Long-Term Conditions
Tender ID: 549910
Tender Details
Tender Description
Together with other GACD member agencies, NHMRC is inviting investigators to submit applications for implementation research that aims to improve the availability of effective, equitable, efficient, integrated, patient-centered, safe, and timely care for people living with multiple long-term conditions including non-communicable disease (MLTC NCD) in low- and middle-income countries (LMICs) and/or in disadvantaged populations in high-income countries (HICs).
For NHMRC funding support in HICs research must focus on Indigenous population groups. In Australia, research must focus on Aboriginal and/or Torres Strait Islander communities.
The objective of the NHMRC-GACD 2024 grant opportunity is to fund implementation research that will generate evidence about when, for whom, and under what circumstances, patient-centered approaches can improve integrated care for patients with MLTC NCD in LMICs and/or for patients in disadvantaged populations in HICs.
The intended outcomes of the NHMRC-GACD 2024 grant opportunity are to fund projects that will collectively: decrease the fragmentation of care for patients living with multiple chronic conditions, and ensure continuity of care across all stages of disease progression; expand the availability of effective, equitable, efficient, integrated, patient-centered, safe, and timely care for patients living with MLTC NCD; reduce health inequities; improve local capacity for implementation research, data collection and harmonization, and stakeholder engagement for MLTC NCD management; and through successful achievement of the above objective, contribute to the United Nations Sustainable Development Goal 3.4 to reduce premature mortality from NCDs by one third by 2030.
