Tender Description
Capital Health Network (CHN) is seeking to commission early intervention activity/activities to support older people to live at home for longer (including those not currently receiving aged care services). The proposed activity/activities should aim to promote healthy ageing, slow decline, and support the ongoing management of chronic conditions.
CHN is inviting proposals from potential providers to:
- co-design and implement targeted interventions to prevent, identify and reduce chronic disease and health issues, avoid inappropriate hospital admissions and improve health outcomes for the elderly and/or;
- support collaborative approaches between multidisciplinary teams and primary care providers and/or.
- expand existing healthy ageing programs where relevant and/or.
- educate primary health care providers on how to connect senior Australians with necessary psychosocial, health, social and welfare supports and/or.
- educate family members or carers on how to manage an older person’s health.

The RFP process commences 4 March 2024.

This Approach to Market is funded utilising Primary Health Network program funding from the Australian Government, Department of Health and Aged Care.
This procurement is open to existing providers under this program as well as new providers.
Activities based in the primary care and community spaces will be prioritised.
