SWSPHN is seeking to commission an eligible organization to establish and deliver the Local Care Teams key component of the Supporting Recovery Pilot Program.

The Supporting Recovery Pilot Program includes 3 key components:

1. Local Care Teams, embedded into existing services to provide individual case and care management and warm referral to trauma-informed services to deliver specific support to victim survivors;

2. Mental health services, including free, long term and trauma-informed mental health care; and

3. Independent evaluation.

The commissioned organization will ensure a holistic service is delivered in partnership with FDSV victim-survivors, key advocates, advisory groups, and family, domestic and sexual violence service providers, and may establish and maintain relationships and partnerships with key FDSV services within SWS.

Services will be trauma informed, person centred, and recovery focused.
