WVPHN is inviting proposals from eligible service providers to implement and deliver the After Hours Program across the WVPHN region.

The After Hours Program will commission service models that increase access to after hours services in western Victoria, by focusing on a place-based commissioning approach to deliver frontline primary health care services and improve the access, delivery, quality and coordination of those services.

The program will address service gaps and will prioritise consumer centred care with a focus on improving service integration and where possible seeking to leverage off existing services, resources and workforce to help build service sustainability. Commissioned services will help meet consumer needs for access to quality after hours primary health care services and help to reduce pressure and non-urgent attendances at EDs across the region
