Tender Description
Healthy North Coast (HNC) is seeking a provider to design an evidence based, regional Integrated Model of Support (IMoS) to improve the care and support provided to residents of Residential Aged Care Facilities (RACH's) at times when GP support is not available, generally referred to as "out of hours support". This model will be designed to 'join up' the systems of care provided by RACH's, Ambulance, Pharmacy, General Practice and Hospitals, so that where appropriate, a clinical response can be provided to residents with deteriorating health conditions, in their home. The goal is that unnecessary hospital admissions can be avoided/reduced.
