Gold Coast Hospital and Health Service (GCHHS) is conducting a procurement activity to establish a standing offer arrangement. The purpose of the activity is to address barriers to discharge resulting in extended patient stays by delivering community based care services that support GCHHS clinical workforce provide in-home care of patients that are in the final stages of care and treatment. Current barriers to discharge that GCHHS seeks to work with community service providers to address and mitigate include:

• Rapid access to respite, convalescence to career support for clients that reside at home.
• Workforce with capacity and capability to be non-acute, high care needs.
• Complex physical needs including (but not limited to) bariatric care, wound care, tracheostomy and stoma care, and enteral feeding.
• Progression of chronic disease
• End of life care
• Challenging behaviors relating to cognitive impairment
• Additional interim and transitional care options
• Services that provide timely support and management of complex social needs, financial issues, legal issues, family conflict resolution/mediation, and guardianship.
• Timely and affordable access to medical, nursing and allied health assessment and management within a community setting.
