Tender Description
Capital Health Network (CHN), the ACT’s Primary Health Network (PHN), is seeking to commission a lead agency to establish and deliver the new Tuggeranong Medicare Mental Health Centre (MMHC). Medicare Mental Health Centres were formerly known as Head to Health centres.
CHN is inviting applications from potential providers to:
- Develop, establish, and operate the Tuggeranong MMHC as an accessible and highly visible entry point to services for people experiencing psychological distress, including person-centred, high-quality, and culturally safe assessment, navigation support, and mental health interventions.
- Provide core service elements outlined by the national Service Model, including:
- Responding to people in significant distress;
- Providing a central point to connect people to other services in the region;
- Providing in-house assessment, including information and support to access services; and,
- Providing evidence-based and evidence-informed immediate support and short-to-medium term episodes of care.
- Engage a skilled multidisciplinary team and build strong relationships with health and wellbeing services and workforce across the region to enable integrated and effective care.
