This procurement relates to the Multicultural and Homelessness funding schedules as outlined by the Department of Health and Aged Care (DoHAC).

SWSPHN is seeking to commission a supplier to deliver Service Navigation Support for CALD, refugee and homeless community members in SWS. The supplier will need to assist the CALD, refugee and homeless community members when accessing primary healthcare services in SWS. The supplier will work in accordance with the SWSPHN's service navigation support model. Consortia applications are welcome.

The key outcomes expected through delivery of the service model include:

• Improved access to primary health care
• Enhanced connection to community health services
• Improved financial relief through subsidized primary care costs
• Enhanced personalized health support for target populations
