ABOUT MURRUMBIDGEE PRIMARY HEALTH NETWORK
Firsthealth is federally funded to deliver the Primary Heath Network program across the Murrumbidgee region. Murrumbidgee Primary Health Network (MPHN) is one of 31 local and independent PHNs established nationally to support the primary healthcare sector and improve health outcomes, particularly for people at risk of poorer health outcomes. We deliver the PHN program across approximately 126,000 square kilometres to more than 250,000 people living in our 508 communities.
With the person at the centre of care, we plan and deliver programs to ensure the needs of our individual local communities inform our approaches. We achieve this by working with our community and partners to:
- Coordinate local services and systems to improve coordination of care.
- Commission primary health services to meet population health needs with a focus on access and equity.
- Support capacity-building to ensure sustainable healthcare workforce through quality improvement, professional development and other innovative measures.

ABOUT THIS REQUEST FOR PROPOSALS (RFP)
The Homelessness Health Support Model of Care adopts a multi-component approach to improve primary care access for those experiencing or at risk of homelessness in the Murrumbidgee region.
Components include:
- The Homelessness Health Support Team
- GP-led Multidisciplinary Team (MDT)
- Pathways for After Hours Access.
The Model of Care aims to improve access to primary health care services specifically for people experiencing homelessness in high priority communities, through outreach and in reach delivery.
This procurement is for the Homelessness Health Support Team Program component of the Model of Care. This component utilises a multidisciplinary team approach to provide in reach and outreach services to target people and families who are currently experiencing homelessness or are at risk of homelessness. This includes (but is not limited to) sleeping rough, couch surfing, and living in overcrowded dwellings.
OPPORTUNITY ON OFFER
MPHN are seeking proposals from suitable providers to establish and deliver the Homelessness Health Support Team component of the Homelessness Health Support Model of Care. The service will provide in reach and outreach services to people experiencing homelessness in identified high priority communities.
This is an open and competitive RFP process.

SUBMISSIONS
Completed responses must be submitted via online submission form. Respondents will receive a Successful Submission Receipt timed and dated upon completion. Respondents are strongly advised to allow adequate time to complete the submission in full. Part submissions will not be accepted, nor will the system be reopened to allow completion.
Submissions must be fully received by the closing date. Murrumbidgee Primary Health Network is not bound to accept the lowest priced Proposal or all or any part of a Proposal and reserves the right to conduct the Request for Proposal process and select the successful respondent(s) as it sees fit. Murrumbidgee Primary Health Network's staff are unable to respond to verbal enquiries in relation to this Request for Proposals. Late proposals will not be accepted.
