Tender Description
The purpose of the development of the framework is:
1. To support Healthy North Coast to measure performance and quality against tangible outcomes
2. To highlight opportunities to identify areas for improvement for Healthy North Coast and individual commissioned services
3. To measure and monitor progress towards achieving the objectives of improving efficiency and effectiveness of medical services for patients and improving coordination of care to ensure patients receive the right care, in the right place, at the right time.
As such, Healthy North Coast is seeking proposals for a suitable provider or organization to design an Integrated Wellbeing Performance and Monitoring Framework that will include, at a minimum, the following:
1. Development of a detailed Project Plan, with clear timelines to undertake the required works
2. Establishment and facilitation of a Project Working Group, to support the implementation of the project, and maintain connection with Healthy North Coast during the delivery of the project
3. Identification of data sources and methods of collection, including frequency for all program areas in the Integrated Wellbeing directorate
4. Performance indicators and data set definitions for all program areas in the Integrated Wellbeing Directorate
5. Responsibilities for acquiring data for all program areas of the Integrated Wellbeing directorate
6. Development of data analysis plans for streams under the Integrated Wellbeing directorate, including Primary Mental Health, Psychosocial Support, Drug and Alcohol Treatment, Suicide Prevention and Aftercare, Community Impact and Children and Young People
7. Development of an Integrated Wellbeing Performance Monitoring Plan
8. Development of a plan for communicating and using performance data and sharing impact
9. Development of dashboards to support monitoring of performance across all 6 streams identified above. The dashboards will need to be maintained by Healthy North Coast, with capability to add additional program areas and data sources regularly.
The framework must align with the following principles:
1. The framework should focus on minimising reporting requirements for PHNs and gathering information that is useful for assessing performance and quality
2. The framework should be clear about what it is measuring and how it will assess performance
3. The framework should measure progress towards outcomes and program objectives, to build a strong picture of the impact of our activities and programs
4. The framework should provide an opportunity to identify continuous quality improvement for the PHN Program and individual PHNs
5. The framework needs to accommodate the different priorities, as they may shift at times, depending on the health needs of our region and policy direction
6. The framework, should include the identification of existing outcomes data that can be used for subsequent evaluation, and at minimum, identify gaps in outcomes data (PROMS, equity, effectiveness, efficiency and efficacy) that are important for subsequent evaluation.
