Melbourne Health invites suitably qualified and experienced aged care providers (Providers) to submit proposals for the provision of bed-based transition care program (TCP) services for Melbourne Health.

Purpose

The purpose of this RFP is to allow Melbourne Health to partner with suitably qualified and experienced Provider(s) that can provide residential bed based TCP to patients within the Melbourne Health catchment area. Melbourne Health is seeking to partner with capacity to provide a total of 23 TCP beds.

Detailed and specific contractual requirements for the provision of the TCP is outlined in the attached Transition Care Services Agreement (Terms and Conditions).

We expect any resulting contract with the preferred Provider(s) to commence from 1 July 2025. We are proposing a contract term of initial three years and two extension options, each option being a one year period, to be exercised at Melbourne Health’s discretion.
