Primary Health Networks (PHNs) have been established with the key objective of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure clients receive the right care in the right place at the right time.
Country to Coast Queensland (CCQ) is an independent, not-for-profit organisation and we proudly deliver the Primary Health Network (PHN) programs on behalf of the Commonwealth Department of Health.
Country to Coast, Qld (CCQ) is funded to deliver the PHASES with Primary Sense project which aims to improve the detection, intervention and management of cardiovascular disease (CVD) within a primary care setting.
By this Request for Proposal, CCQ seeks to engage a benefits realisation and evaluation partner to develop benefit metrics, evaluate and report on the PHASES with Primary Sense project including implementation, operation and the outcomes that have been achieved. The evaluation is over a 5-year period, commencing November/December 2024 until FY2028/2029.
