Firsthealth is federally funded to deliver the Primary Health Network program across the Murrumbidgee region. Murrumbidgee Primary Health Network (MPHN) is one of 31 local and independent PHNs established nationally to support the primary healthcare sector and improve health outcomes, particularly for people at risk of poorer health outcomes. We deliver the PHN program across approximately 126,000 square kilometres to more than 250,000 people living in our 508 communities.

ABOUT THIS REQUEST FOR PROPOSALS (RFP)

MPHN received grant funding under the General Practice in Aged Care Incentive - Thin Markets initiative from the Department of Health and Aged Care (DoHAC) in early 2024. The grant is intended to provide needs-based funding to support innovative and local solutions to improve access to timely primary care for permanent residents of Residential Aged Care Homes (RACHs) in communities identified as having thin primary care markets (i.e., communities recognised to be underserviced by primary care).

The Murrumbidgee Aged Care Access Network (MACAN) Model has been developed in consultation with relevant stakeholders to deliver consistent, coordinated and high-quality primary care to permanent residents of RACHs in communities identified as having thin markets.

OPPORTUNITY ON OFFER

Murrumbidgee Primary Health Network (MPHN) is seeking proposals from suitably qualified Nurse Practitioners and/or General Practitioners to deliver the primary care support service component of the MACAN Model. The Nurse Practitioner and/or General Practitioner (NP/GP Provider) will collaborate with MPHNs General Practice and Aged Care Incentive Coordinator and the residents' regular primary care team to provide proactive and responsive care to residents of RACH's in identified thin market communities.

Murrumbidgee Primary Health Network is not bound to accept the lowest priced Proposal or all or any part of a Proposal and reserves the right to conduct the Request for Proposal process and select the successful respondent(s) as it sees fit. Murrumbidgee Primary Health Network's staff are unable to respond to verbal enquiries in relation to this Request for Proposals. Late proposals will not be accepted.
