ABOUT: Sydney North Health Network (SNHN) is inviting suitably qualified and experienced organisations to apply to deliver innovative and creative approaches to support multi-disciplinary care in General Practice in Northern Sydney through a 'Request for Proposal' process. The proposal will aim to increase primary health care access for patients by providing General Practitioners with multi-disciplinary support to manage priority populations, improve the management of chronic conditions and reduce avoidable hospitalisations. This service will be delivered in the Northern Sydney region, specifically in the Ryde and Hornsby areas.
