COORDINARE - South Eastern NSW Primary Health Network (SENSW PHN) invites grant applications from local organisations or services to apply for funding to support existing activities that contribute to the health and wellbeing of vulnerable communities

The purposes of these grants are to enhance services that address the social determinants of health through clinical service delivery, health and wellbeing education, service navigation and other supports that promote engagement with preventative and primary healthcare.

A total of $400,000 excluding GST will be dispersed through this initiative, with grants of up to $80,000 to be distributed to each successful applicant.

There will be an online industry briefing and Q&A session on Monday 21 October at 3pm (AEDT). Please RSVP to attend the briefing by emailing commissioning@coordinare.org.au.
