Gippsland PHN has issued this Request for Tender (RFT) for the purpose of inviting suitably qualified and experienced organisations (Tenderers) to lodge a Tender (Tender) to provide consultant and evaluation support services for a Multi-Disciplinary Team (MDT).

These services are required to support the successful implementation of a multi-disciplinary team capability into a Latrobe based general practice with a specific focus on patients with and at risk of heart failure.

Contracts are expected to be awarded in December 2024 until 30 June 2025.

The total budget for this procurement is up to approximately $150k ex GST.

Tenderers should address the evelaution criteria detailed in the specifications. A completed Tender Application Form, along with a Project Plan and any other relevant documentation must be uploaded to Gippsland PHN's online forum portal by the closing date of 5.00pm on Monday 11 November 2024.
