Primary Health Networks (PHNs) have been established with the key objective of increasing the efficiency and effectiveness of health services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure clients receive the right care in the right place at the right time.
Country to Coast Queensland (CCQ) is an independent, not-for-profit organisation and we proudly deliver the Primary Health Network (PHN) programs on behalf of the Commonwealth Department of Health.
CCQ is seeking to commission a Nurse Practitioner model of care in the region over the next three years. Specifically, funding available through the opportunity will enable the commissioning of two full-time equivalent (FTE) Nurse Practitioners across the region including Biloela (0.5FTE), Emerald (0.5FTE), Tin Can Bay (0.5FTE) Gympie (0.5FTE). Ultimately, this initiative aims to enhance access to primary care services for aged care residents and improve overall health outcomes.
CCQ will engage a provider or multiple providers from February 2025 to June 2027 to deliver Nurse Practitioner (NP) services within Residential Aged Care Homes in set locations identified as having significant primary care access challenges. The primary goal of the service will be to meet the clinical needs of residents in identified RACHs. Additional objectives include enhancing collaboration and relationships across the multidisciplinary care team, uplifting the sectors understanding of the NP role within the aged care setting, providing capacity building to staff within RACHs through education and mentorship, and supporting the utilisation and integration of digital health initiatives for streamlined care.
General Practices or Residential Aged Care Homes located in Tin Can Bay, Gympie, Emerald and/or Biloela, or Private Practice Nurse Practitioners/ Nurse Practitioners employed by community organisations are invited to apply.
