WVPHN is inviting proposals from eligible service providers to implement and deliver the MDT Care Program, targeting areas in most need within the WVPHN region.

The MDT Care Program will see the commissioning of service models that strengthen health care for people with complex chronic healthcare needs. The service models will achieve this by focusing on a place-based commissioning approach to deliver services, and improve the access, delivery, quality and coordination of those services. The program prioritises a person-centred approach through formal or informal partnerships with a focus on service integration and care coordination, and supports opportunities to leverage existing service offerings, resources and workforce in order to develop a plan for long-term sustainability.

Based on the findings from the research and the co-design process, commissioned service models will be targeted to support consumers with complex chronic condition health needs to navigate the service system and access required supporting allied health services. Commissioned service models will need to incorporate both of the following:

1. Allied health service provision that is integrated within primary care settings to increase availability in the Wimmera Grampians region for consumers with complex chronic health needs; and

2. Dedicated care coordination that is delivered within a primary care setting, to support people with complex chronic condition health needs, and who are at high risk of deterioration to navigate the service system and access required supporting allied health services.
