Tender Description

CESPHN is inviting suitably qualified and experienced organisations to apply to deliver a program to improve access to timely and integrated GP care for residents in RACHs (Residential Aged Care Homes) through a 'Request for Proposal' process. The proposal aims to address gaps in GP service arrangements for residents by providing short term access to

GP care in situations where the resident's usual GP or an alternative cannot be sourced such as when:

  1. A RACH resident's regular GP is no longer available to provide ongoing care.
  2. A RACH resident's regular GP takes short-term planned or unplanned leave, and the GP's usual deputing service is unavailable.

CESPHN requires the service to:

  1. Be provided to the whole of the CESPHN region,
  2. Be available 5 days a week (Monday-Friday 8:00am - 6:00pm) with view to extension to 7 day a week cover through weekend on-call service in the second year.
  3. Have one centralised point of contact (designated phone line) for RACHs, GP's, hospitals to access.
  4. Provide temporary short-term (up to 8 weeks) GP care to residents while linking and matching them to a regular GP for ongoing continuity of care in consultation with the resident/carer and RACH
  5. Provide the service in alignment with the goals and aims of the new GP Aged Care Incentive (insert link to this document), by supporting regular GPs to follow their patients into RACHs and assisting new GPs connect with their residents.
  6. Promote service integration and coordination with local GPs and GP practices already providing care to residents in RACHs, through models that favour use of regular GPs in the RACH covering the same regions.

The  portal contains questions that require responses for the submission to be considered. The application should capture in Part C all the relevant financial components that are in scope for funding.

Only responses submitted via the online form will be considered for evaluation. Please note, you will not be able to upload a word version of your submission.
