Tender Description

Country to Coast Queensland (CCQ) delivers the PHN program across Central Queensland, Wide Bay, and Sunshine Coast, guided by the vision of supporting the region to live its healthiest lives. CCQ is dedicated to enhancing resilience of primary healthcare and communities amidst a changing climate and disasters through strategic planning, targeted programs, and robust partnerships.

CCQ, in partnership with Australian Business Volunteers, has secured funding from the National Emergency Management Agency's (NEMA) Disaster Ready Fund Round 2 to deliver the Resilient Care Program 2025-2027. This program aims to enhance the resilience of primary healthcare and communities in five Local Government Areas: Livingstone, Rockhampton, Gladstone, Bundaberg, and Fraser Coast.

The Resilient Care Program focuses on three core outcomes:

- Strengthening Resilience: Enhance the capacity of primary healthcare providers and communities to respond to the challenges of a changing climate.

- Improving Integration: Embed primary healthcare providers and communities into local and district disaster management systems to improve preparedness, response, and recovery efforts.

- Sharing Insights: Document and share actionable insights with other PHNs and stakeholders to strengthen the resilience of primary healthcare systems nationwide.

Resilient Care will employ action learning methodology, focused on discovering and refining optimal approaches to achieve objectives. A comprehensive Monitoring, Evaluation, and Learning (MEL) framework will assess the program's outcomes and the interconnected impacts across all five sites.

CCQ invites proposals from qualified providers or consortia to lead the Monitoring, Evaluation, and Learning (MEL) components of the Resilient Care Program.

Scope of Work includes:

  • Build upon existing evidence and best practices to inform program delivery.
  • Create a framework to evaluate program outcomes and the interconnected impacts of strategies implemented across five sites.
  • Facilitate adaptive learning processes to continuously improve program design and implementation.
  • Document and disseminate findings, tools, and methodologies to support nationwide resilience-building efforts.
